The Lytic Cocktails

The lytic cocktails contains nitroglycerine  50-100 micrograms, heparin 5000 units for diagnostic procedure and  8,000 – 10,000 units for coronary intervention and diltiazem 5 mg. Some operators use 3,000 units of heparin. However,  the incidence of radial occlusion  is more with 3,000 units of heparin than where 5,000 units were used.  Heparin may be given intravenously or intra arterially, however intra arterial heparin may cause local irritation  as it is acidic. Lytic cocktails should always be introduced after proper mixing  with blood, so that  local irritation and warm feeling in hand  is less.
